After you are scheduled you will be sent a link to create an online portal.  You will upload pictures of your insurance card, photo ID and fill out additional paperwork prior to your first appointment. Your first appointment can not be held until the paperwork is completed. 

Monday & Wednesday 8:30 am - 4:30 pm (Telehealth only)

Tuesday & Thursday 9:00 am - 4:00 pm (In-person or Telehealth)

Every other Friday 9:30 am - 3:30 pm
